Medicare Advantage at The Polyclinic
We want to keep you healthy, out of the hospital, and doing the things you love most. In our Medicare Advantage program, we coordinate your care in a personalized and comprehensive way, collaborating with your primary care physician and insurance company. We offer multiple Medicare Advantage plans to give you options on what fits your needs.
What is Medicare Advantage?
Medicare Advantage plans (Part C) provide the same hospital and medical benefits as Original Medicare (Part A and Part B) and can provide additional benefits like vision and dental – even gym memberships. Medicare Advantage plans bundle your benefits into one plan with one card from one insurance company, often offering a more cost effective option.

How do I get started?
If you are turning 65:
You have seven months to enroll: three months prior to your 65th birthday; your birthday month and three months after.
If you are over 65:
You can enroll in a Medicare Advantage plan during the Open Enrollment Period from October 15-December 7 each year unless you qualify for a special enrollment period (SEP). Review your plan each year to make sure it still meets your health needs. Polyclinic patients can access our Medicare resources even after turning 65.

The Polyclinic accepts retirement benefit plans from state or federal governments or private employers. Please check our accepted insurance page or attend our Polyclinic, Medicare, and You class to find the right plan for you.
The Polyclinic does not accept Medicare Advantage Private Fee for Service (PFFS) plans. This is not a complete listing of plans available in your service area.
