The Polyclinic, dba Optum Care Washington, will stop accepting Apple Health UnitedHealthcare Community plans on 1/1/25. Optum Care Washington will continue accepting United Healthcare Medicare Advantage D-SNP plans.
We work with nearly every major insurance company in the region. But please confirm with your health insurer that your specific plan includes Optum Care Washington. This will help make sure there are no surprises about your coverage or any costs to you.
Questions to ask your insurer
- Are Optum Care Washington doctors I want to see in my network?
- What hospitals are included in my network?
- What are my copays for primary and specialty care visits?
- What preventive care services or wellness visits are covered?
- Do I need a referral from my primary care doctor to see a specialist?
- Do I need advance authorization for any tests, medications or procedures, including surgery?
Key dates and contact information
Open enrollment dates for insurance coverage:
- Public and private employers: Dates vary, but usually in November and December
- Washington Health Benefit Exchange: Nov. 1–Jan. 31 (you must sign up by Dec. 15, for coverage started Jan. 1)
- Medicare: Oct. 15–Dec. 7

Medicare and Medicare Advantage plans
Effective January 1, 2025, Premera is leaving the Medicare Advantage Plan entirely in Washington state. Therefore, The Polyclinic, doing business as, Optum Care Washington can no longer accept the Premera Medicare Advantage plan. We still accept Original Medicare and other Medicare Advantage plans.
