Connected Care Program - Remote Patient Monitoring
The Polyclinic now offers remote patient monitoring (Connected Care Program) through the Vivify Health smartphone application for select patients at higher risk for chronic conditions. This is one way for you to be proactive in your health and stay connected with your care team from the safety of your home.
How it works
The Connected Care monitoring enables you to self-report your health status via a smartphone application and quickly, easily, and securely share that information with your health care provider. We’ll be alerted to a change in your health status and can intervene, if necessary.
Right now, this service is available for select Polyclinic patients that are higher risk for chronic conditions. These selected patients will receive a text message from Vivify inviting you to enroll in remote monitoring.
• The text message will look like the following: “Hi (First Name). You are invited to participate in The Polyclinic's Connected Care program. A letter was sent to you explaining the program. Reply STOP at any time to stop. Tap the link to start…”
• When you receive the text message from Vivify, please tap on the download option and begin your screening. This will only take a few moments and will begin the remote monitoring process.
• Included are instructions to download and enroll the Vivify smartphone application. Please wait to enroll until you receive the text message from Vivify.
Once you’re enrolled you will be asked a series of screening questions daily. If you give an answer that might indicate a change in your health status, you will be contacted by a remote nursing team who will screen you and discuss recommended next steps for your care. This includes escalating care to your Polyclinic provider. If you have a concern at any time, you can also request real-time responses from the centralized monitoring team 24/7.
