Providing Care You Need, When You Need It
Polyclinic patients receiving care in a Skilled Nursing Facility can continue to receive care from The Polyclinic without changing insurance coverage.
The Polyclinic Extended Care Team coordinates care through visits on your behalf in many facilities in the Puget Sound area. These visits are billed through your insurance company the same as they would with a doctor’s office visit.
As part of your primary care team, our Extended Care nurse practitioners function as a part of your primary care team. After each visit and at discharge, they communicate with your physician and your family about your progress and care. They are available to provide care in many locations such as:
- Skilled Nursing Facility (SNF)
- Assisted Living
- Adult Family Home
- Your private home
You may be approached by insurance representatives with a suggestion that you need a new health plan now that you are in a SNF. The Polyclinic Extended Care Team will continue to care for you under your current insurance plan.
Extended Care Team Nurse Practitioners
Nurse Practitioners, many of which are ARNPs or Advanced Registered Nurse Practitioners, are Registered Nurses with either a Master Degree or Doctorate Degree in Nursing. ARNPs are licensed with the State of Washington, and offer personalized care and are trained to examine and establish medical diagnoses and treatment plans for acute and chronic health conditions.
Extended Care Team Members
Katie Nega MSN, ARNP, GNP-BC
Katie has been a nurse since 2001 and a nurse practitioner since 2008. She is board certified by the American Nurses Credentialing Center (ANCC). Katie has extensive experience working in skilled nursing and long term facilities. Her clinical interests include dementia, wound care, nutrition and end-of-life care. She enjoys the opportunity to empower and teach patients and their families through education.
Marilee Jensen MN, ARNP
Marilee has more than 25 years of clinical experience and 15 years of leading clinical education. Prior to becoming a nurse practitioner, she worked in critical care nursing for 15 years. Marilee focuses on providing quality, compassionate care for elderly and complex patients. Through the Extended Care program, Marilee partners with physicians to ensure coordinated care when patients transition to and from a nursing facility.
