We offer our patients with type 1 diabetes many visit types to meet their needs. Whether you have been living with type 1 since adolescence or were diagnosed later in life, our certified diabetes educators will work closely with you and your physician to help manage your care.
Below is a list of appointment types offered for type 1 patients and what they each entail; all visits are with a certified diabetes educator.
Continuous Glucose Monitoring (CGM)
CGM is a way to measure glucose levels in real-time 24 hours a day. A glucose sensor is inserted under the skin to measure glucose levels in tissue fluid. It connects to a transmitter that wirelessly sends the data to a device for monitoring and display. There are two types of appointments for CGM:
- Clinical Device
Two appointments, one week apart.
1st visit: 30 minutes / 2nd visit: 60 minutes
- Personal Device
Two appointments, one week apart
1st visit: 90 minutes / 2nd visit: 60 minutes
An insulin pump is a small computerized device used to help patients manage their blood sugar. The pump delivers short-acting insulin continuously throughout the day as opposed to giving manual injections. The Polyclinic is contracted with the following insulin pump companies to provide device training: Animas, Medtronic, OmniPod, and Tandem. All of our diabetes educators are certified trainers with each company to ensure quality and safety.
- Starting a New Device (New/Different Brand)
Two 90 minute appointments, scheduled one to two days apart
- Upgrading an Existing Device (Same Brand)
One 90 minute appointment
- Insulin Pump with CGM Integration

Insulin or Other Injectable Therapy
Two appointments, scheduled one week apart
1st visit: 30 minutes / 2nd visit: 30 minutes
Whether you start a new or different type insulin, or add a new line of injectable therapy, our diabetes educators will review techniques, how the medication works in the body, and how to recognize and treat hypoglycemia (low blood sugar).
Medical Nutrition Therapy
Consult: 60 minutes / Follow-ups: 30 minutes
Our registered dietitians provide nutrition counseling to help patients make healthy food choices and lifestyle changes to reduce the risk of complications. This team takes into account your individual nutrition needs, as well as taking any cultural or religious preferences. All of our dietitians are also certified diabetes educators, providing additional expertise on managing diabetes.
