Annual enrollment for the Washington Health Benefit Exchange begins November 1, 2018. If you’re considering enrolling in an insurance plan available through the exchange (www.wahealthplanfinder.org ) and want to see Polyclinic providers in 2019, you will need to select one of the plans below:

Premera Blue Cross PersonalCare - Includes Primary and Specialty Care

The Polyclinic participates in this plan through the Virginia Mason network. Patients on Premera PersonalCare can see any Polyclinic primary care and specialty care providers. It’s important to note that the in-network hospital for this plan is Virginia Mason Medical Center First Hill. Any patient needing emergency room or inpatient care will need to go to Virginia Mason and will be treated by Virginia Mason providers or Polyclinic providers as available.

One exception for inpatient care is obstetrics. Because Virginia Mason doesn’t offer obstetric care, Polyclinic OB patients may deliver at Swedish Medical Center First Hill or Northwest Hospital.

Kaiser Permanente Core and Flex Plans - Includes Specialty Care Only; Referral Required

The Polyclinic is in-network for specialty care only on Kaiser’s Core and Flex Plans. The Polyclinic is NOT in-network for primary care. Patients can see any Polyclinic specialists with a referral from your Kaiser Permanente primary care provider.
