At The Polyclinic Anticoagulation Clinic (ACC), we help patients manage anticoagulation therapy known as warfarin or Coumadin®.
Anticoagulants (sometimes called "blood thinners") are medications that slow down the blood clotting process. Angicoagulants can keep harmful clots from forming in your heart, veins or arteries. These blood clots can block the flow of blood causing deep vein thrombosis (DVT), pulmonary embolus (PE), heart attacks or strokes.
The most common indication for anticoagulation therapy is atrial fibrillation, a type of irregular heart rhythm.
